Practical video consent

For the practical assignment you upload a video of a training moment. Because people may be recognisable in that footage, we ask for this consent.

For the practical assignment of the FBC course you upload a video of a training moment. Because people may appear recognisably in that footage, possibly including underage players, we ask for the consent below. This statement forms part of our Privacy Statement.

Part A. Consent of the participant, required when uploading

By uploading the video you declare and agree to the following:

  1. You consent to the storage, viewing and assessment of your video submission and of your own image and voice appearing in it, by FBC and the assessors engaged by FBC.
  2. The video is used solely for the assessment of your practical assignment and the related quality control. Not for marketing, unless you give separate and explicit consent for that in Part D.
  3. The video is stored with our video service (Bunny Stream, EU storage) with limited, secured access. No public or downloadable distribution takes place.
  4. Retention period: 6 months after the final assessment of your practical assignment. After that we delete the video.
  5. You may withdraw your consent at any time via info@footballbraincoaching.com. We then delete the video, unless a statutory retention obligation requires otherwise. Withdrawal may affect the assessment.

Part B. People appearing in the footage, required statement

  1. You are responsible for making sure that the people who appear recognisably in your video are aware of this and have agreed to it.
  2. Do minors under the age of 16 appear recognisably in the footage? Then, before uploading, you must have obtained the consent of their parents or guardians, using the form in Part C.
  • No minors appear recognisably in the footage.
  • Minors do appear and I have obtained parental consent for them in line with Part C and keep it on file.

Part C. Model form for parental consent

The coach has this filled in by the parent or guardian for each minor involved and keeps it on file. FBC does not receive this form as a matter of course, but may ask for it in case of doubt.

Consent for filming for the FBC practical assignment

  • Name of the child: ……………… · Date of birth: …………

  • Purpose: a short training video that the coach submits for the assessment of their FBC course. The video is used only for that assessment, stored securely and deleted no later than 6 months after the assessment.
  • As a parent or guardian I consent to my child appearing recognisably in this training video, solely for the purpose stated above. I understand that I can withdraw this consent.

Part D. Optional: consent for promotional use

This consent is separate and not required. You can complete the practical assignment without giving it.

  • I also consent to parts of my video being used by FBC for promotional purposes.

For minors appearing in the footage this requires separate parental consent.
